There are many reasons someone may want to legally change their name during or as a result of a family law matter. If you have been remarried, widowed or divorced and would like to change your name as an adult, it is a fairly simple process. An adult name change will require a criminal background check unless it is being made as part of a divorce action. You will need one of the following documents:

  • Divorce decree (you can also request a name change as part of the divorce process)
  • Spouseโ€™s death certificate
